NDC for Petacel Mapping to actHIB

Hello,
I received a question regarding the mapping of NDC code 49281054515. The CDC has an NDC crosswalk file to crosswalk between the NDC codes that providers buy vs. the NDC on the individual vaccine (i.e. the unit that is used) - you can either use online or download the tables

I have attached some screen prints for ActHIB and Pentacel from the online lookup – the screen print for ActHIB shows the column names since it is at the top – the screen print for Pentacel uses the same column names but you can’t see them (the first column is the NDC for sale and the 8th column is the NDC for use)

The mapping for ActHIB is okay but for Pentacel, the NDC code 49281054515 is listed as ActHIB in OMOP Vocabulary (I have circled this NDC in the screen print word document) – this NDC is listed as Pentacel.

Is this an incorrect mapping or is there a reason that the NDC code is mapping to ActHIB?

Pentacel consists of a liquid vaccine component (DTaP-IPV component) and
a lyophilized vaccine component (ActHIB vaccine).
Here
Each component vaccine of a multiple component vaccine will have a unique NDC which also differs from the NDC of the unit of sale. Pentacel is an example. This means that there will be two rows linking the unit of use to unit of sale.
49281-0510-05 = Pentacel
49281-0545-15 = ActHIB
49281-0560-05 = DTaP-IPV

Which means OHDSI vocabulary is correct.

Dima, Thanks for your response. Since Meghan is out, I was looking into this thread in her absence. Pardon my ignorance here since I dont have much knowledge of the mapping area.

Per the crosswalk, the use NDC code for ‘Pentacel’ is 49281-0545-15.

Seems, there’s a misentepretation how this crosswalk works:
First column (Sale NDC11) stands for Pentacel

Which Maps to
http://athena.ohdsi.org/search-terms/terms/19131879 (you can see in synonym that it’s Pentacel).

“Use NDC11” column stands for its components,
which are mapped to http://athena.ohdsi.org/search-terms/terms/42800807 - ActHIB
and
http://athena.ohdsi.org/search-terms/terms/19131877 - DTaP-IPV/Hib

According to Pentacel leaflet
 Pentacel consists of a liquid vaccine component (DTaP-IPV component) and
a lyophilized vaccine component (ActHIB vaccine).
So, the second component in CVX crosswalk should be “DTaP-IPV”, not a “DTaP-IPV/Hib” as stated there.
And another confusion is that you have DTaP-Hib-IPV near the 49281054515 code (which is kinda wrong)

Please let me know your use case, so we can figure out the best solution, since the NDC - CVX crosswalk seems to be confusing in this case.

Yes, ActHIB relates to the 49281054515.
To avoid this confusion you need to look separately on NDC Unit of Use and NDC Unit of Sale tables.

Based on this spreadsheet you shared with me (Unit of Sale vs Unit of Use). I see that 49281-510-05 maps to PENTACEL in Unit of Sale since this is the vaccine. This makes sense.
I also see that 49281-545-15 maps to ActHIB in Unit of Use which makes sense as its a component of the vaccine. But I also see that 49281-560-05 maps to PENTACEL in Unit of Use. Which does not match our mapping of 49281-560-05 to DTaP-IPV. I don’t see 49281-560-05 mapped to DTaP-IPV in Unit of Use.

Anyway, 49281-560-05 is Pentacel
and this concept doesn’t exist in OHDSI Vocabulary, so, you need to add it to your concept set.

Is 49281-560-05 included in your list of concepts to add?

Also want to add that the business case the data owner is stating here is that the use ndc code 49281-545-15 is being used for Pentacel because the EMR dataset uses the ndc codes (Unit of use) given to the patient – not what the provider/practice (Sale of use) bought. I suppose that’s where the disconnect is coming from.

49281-545-15 (ActHib) is a required component of Pentacel. That’s why you can use 49281-545-15 for Pentacel, I suppose.
Do you have 49281-0560-05 code along with 49281-545-15 in a patient data?

If you mean patient examples then I will have to look for them. I did not receive any from the data owner.
