Where to find code for constructing _era tables?

I’m looking to create the _era tables in the All of Us research program data (they are not populated in All of us) but I’m having trouble finding this SQL code so I can modify it to work in All of Us. We do not have any write access to the db, so the code will need to be customized. I found a few links in older forum posts, but they mostly appear to be broken or outdated.
