Vocabulary for annotating surveys

What vocabularies are used for annotating quality of life and other standard survey instruments? I am particularly interested in the following instruments:

EORTC QLQ-C30 - quality of life of cancer patients
http://groups.eortc.be/qol/eortc-qlq-c30

PRO-CTCAE - patient reported outcomes of adverse events
http://healthcaredelivery.cancer.gov/pro-ctcae/

S-CAHPS - Consumer Assessment of Healthcare Providers and Systems Surgical Care Survey

Looks like we have to add them. LOINC has a bunch of surveys:

select * from concept where concept_class_id=‘Survey’;

But not the one you mention.
