How to represent Clinical Reported Outcomes from forms?

Our current use case is based on patient-reported outcomes, collected by surveys which we save under Survey Conduct table in CDM v6.0. The questions and answers are saved as observations, according to this discussion SURVEY data in OMOP CDM - updated · Issue #137 · OHDSI/CommonDataModel · GitHub

However, we have an edge case which are Clinical-reported Outcomes, basically observations collected via forms which are filled not by the patient, but by the clinician itself, about several things: patient state, surgery details…

The SURVEY_CONDUCT documentation does not explicitly say this, but it hints at the fact that these are patient-reported (even if someone else is responding for them i.e. respondent_type_concept_id). Does it make sense to use this table for CROs as well or should they live as separate observations?
