How to record chromosome name and position in the G-CDM tables?

I have NGS variance calling records I want to covert into records in G-CDM tables (https://github.com/OHDSI/Genomic-CDM/). However, I cannot find the corresponding columns for mapping the chromosome name and position which are the required fields in the VCF files.

Is it possible to map these fields?
