How to include the Health Examination data at Visit table?

These days, regular health examination is quite popular.
Especially in Korea, the government mandates every citizens have health examination per every other year. The government and employer pay for the examination.
The health examination includes history taking, ROS (review of system), physical examination, radiology test (simple X-ray, ultrasonography), endocsopy (EGD), colonoscopy, laboratory test. The kinds of examination depends on age and sex of the recipients. Thus, health examination data take a lot in our EHR data (more than millions of records).
However, in the Visits domain (OMOP visit concepts - vocabulary id 24), there are only four concepts: Inpatient Visit, Outpatient Visit, Emergency Room Visit, and Long Term Care Visit.
The recipients of health examination are quite different from any of above visits. Because the reason to visit hospital is not a disease.
Do we need to add new concept for health examination in vocabulary id 24, or is there any other way to handle this?

In the US, the health examination is generally part of an outpatient visit to the primary care physician. Does outpatient visit mean something different in Korea?

In the U.S., it is common for physicians to perform regular screening physicals in the outpatient setting, even in the absence of symptoms or illness. However, we are seeing more “Health Screenings” (Health Risk Assessment) happening at health fairs, places of employment, etc, which are not at the doctors office.

So while I think all of these could simply be labeled as “Outpatient” there may be some level of granularity that could be expanded upon (Outpatient Clinic, Outpatient Other, etc).

This raises an interesting issue. In us, the general health examination is a procedure, coded as cpt4 for reimbursement purposes. The visit is essentially the setting where the procedure takes place. How are general health exams captured in other systems? Are they procedures elsewhere, or are only observations within the exam recorded, or are they a different type of entity entirely?

I think health examination has different clinical meaning from outpatient visit, because they are not sick. By separating recipients of health examination from outpatient and inpatient visit, we can get data of normal or super-normal control.

I would like clarify the term ‘health examination’. Does regular health check-up is right for my claim?

It should be a procedure. If we want to indicate the special circumstance of the data generation, we could put that into the procedure_type_concept_id.

It is, Rae. However, what you have is a special formalized health checkup, not just what a doctor might decide to do from time to time.
