Health systems with EHR data in OMOP

I’m putting together a list of organizations with EHR data from their health system in OMOP. If you know of an organization with an OMOP instance of their EHR data, please let me know:

  1. Name of the organization/health system
  2. Latitude/longitude
  3. Website with information about the OMOP CDM at that organization.

Currently have ~40 confirmed organizations…
