Gleason score best practice

Hello everyone! I’m building a CDM regarding EHR data of patients with prostate cancer and i have come across to many Gleason scores measurements. What is the best practice regarding Gleason measurements?
I have 2 ways in mind:

  1. The concept id 734332 (Gleason finding) as measurement_concept_id and 1633596 (Gleason Score 4+3) as value_as_concept_id.
  2. The concept_id 1633596 (Gleason Score 4+3) as measurement_concept_id and nothing as value

It’s a small detail but I want to understand the best practice.

Hi @manoshatzak. The conventions for Cancer Modifiers are a work in progress. Personally, I would vote for your option 1.

Please see also the de facto convention for linking the measurement to the CONDITION_OCCURRENCE record of the prostate cancer diagnosis as described here: Oncology Conventions. Specifically, the MEASUREMENT record set:

  • meas_event_field_concept_id = 1147127 – static for all condition modifiers
  • measurement_event_id = the condition_occurrence_id to which this Cancer Modifier refers

This permits identification of all Cancer Modifiers that relate to the same CONDITION_OCCURRENCE.
