I was wondering why the ICD-10 code D00.0: “Carcinoma in situ: Lip, oral cavity and pharynx” maps to SNOMED concept ID 4113107: “Carcinoma of lip, oral cavity and pharynx”. Souldn’t it map to another “in situ” concept ID instead of an ID concept under malignant neoplasms? D00.0 is not (yet) malignant, whereas the SNOMED concept ID 4113107 is. D00.0 could for instance be mapped to “Carcinoma in situ of mouth” (concept ID 4243266).
Thank you for your thoughtful observation. You are absolutely correct that the distinction between carcinoma in situ and malignant neoplasms is clinically significant.
We appreciate your suggestion to map ICD-10 code D00.0 to a more appropriate SNOMED CT concept, such as “Carcinoma in situ of mouth” (concept ID 4243266), rather than “Carcinoma of lip, oral cavity and pharynx” (concept ID 4113107). This would better reflect the non-malignant nature of the condition while maintaining anatomical accuracy.
Your feedback is valuable, this mapping will be reviewed and corrected in the Feb 2025 release.
Friends:
Yes, we need to fix the cis concepts. Unfortunately, ICDO and SNOMED handles them differently. We need to figure out what we do. Come to the Onco WG.
