Clinical Note Extract (NLP)

Hello,

I am hoping someone can help me understand how the NLP table might look or how one might structure clinical notes.

I can only seem to find examples of a longer, paragraph like Clinical note turning into a summarized and bulleted cleaner note.

Has anyone had experience converting a long unstructured note, containing Procedures, diagnoses, imaging, etc. into a table rather than a summarized note?

Apologies if this isn’t well written, Its hard to describe and hard to find any concrete examples of final results… Thanks
