Case Definitions for Chronic Conditions Using SNOMED/OMOP codes?

Hello All!

I am a prevention effectiveness fellow doing research on chronic conditions. I am looking for case definitions of chronic conditions based on SNOMED or OMOP codes.

Does anyone on here happen to know where I may be able to find either
(1) case definitions using SNOMED / OMOP
or
(2) just a list of translated codes from ICD to SNOMED / OMOP for chronic conditions?

I am mostly interested in conditions like hypertension, asthma, diabetes, sleep apnea, dyslipidemia, depression, NAFLD, etc.

This would save me a lot of the valuable time I have left in my fellowship. Thank you in advance!

You can use the list of ICD10CM concepts from here:
https://www.hcup-us.ahrq.gov/toolssoftware/chronic_icd10/chronic_icd10.jsp#download
and then use the mappings taken from concept_relationship table to get the corresponding SNOMED concepts.
Note, this list isn’t perfect, there’re some concepts that are not chronic obviously:
S78022A’ Partial traumatic amputation at left hip joint, initial encounter ,
‘M86052’ Acute hematogenous osteomyelitis, left femur
So you exclude all the S and M chapters and it should be totally fine then.

Another idea - to define chronic diseases using SNOMED attribute relationship = ‘Has clinical course’, unfortunately doesn’t work as a lot of chronic conditions don’t have it.
for example, ‘Diabetes mellitus’ doesn’t have such a relationship:
