For any concept in atlas:
Does RC count include *_concept_id occurrences (in the corresponding domain table)?
or value_as_concept_id occurrences as well (for instance in observation table)?

yes, the RC is referring to the domain concept_id (condition_concept_id, drug_concept_id, etc) not any of the secondary columns that may also reference a concept ID.
