I seem to be having the same issue but I’m using MS Sql Server. I’m checking the versions of the jdbc jar now. I’ll let you know how I make out. Thank you, Thank you, Thank you for the very helpful post 
Full Tomcat output is attached (file is not pdf, I renamed it to get around the security check, rename to log.txt to read).

I found this near the top of the logs:
Unable to create schema [OHDSI_schema]
