The concept set defines the list of codes that will be used to find records in the CDM, and these codes are specified as CDM concept IDs, and you can declare if they should include descendants from the Vocabulary, and if the concepts should be used for exclusions from the final concept set result. So, when a cohort definition is materialized into the patient-level start and end dates, it uses the vocabulary to find the child-conceptIDs defined in the concept set, as well as the list of concepts that should be excluded from being included in the code list.
